Are You A Carer?

A carer is someone who provides support to family or friends who could not manage without this help. This could be caring for a relative, a partner or friend who may be suffering from illness, frailty, disability, mental health or substance misuse issues. It includes young people who may be caring at home.

Online Services For The Person You Care For

If you are a carer for somebody at the surgery, you may find it useful to sign up to online services to manage their appointments, order medication, view test results online etc. The Family Surgery offers proxy access to medical records, this is where somebody is given access to another person's medical record (i.e. the carer is given access to the medical records of the person they care for).

Please find more information about how proxy access works here

If this is something that sounds useful to you, please fill in our proxy access form which can be found here

Please note that we need consent/permission from the patient you are caring for before access can be granted.
